Organizations Medicare Abuse

Health care at home is on the rise replacing expensive hospital visitations and stays. Instead, the care comes to the patient at their own location and the company gets reimbursed by Medicare. This substitution holds lots of benefits for both the patient and the country as it has potential to save billions every year as well as keeping hospitals rooms open. The leading company in right now in the field of home visits is Amedisys and they derive 90% of their revenue from Medicare abuse.
This type of practice of course carries with it questions and concerns. With home visits, there is little supervision over these companies and doctors, as well as few overlying rules to account for pricing and charges. With no one keeping a close eye on the each and every check up like it is done in a hospital, it is hard to tell if the patients are getting exactly what they need, or if they are even getting what it is said they are getting. It is hard to tell if their treatment is the full package, or are they getting shorthanded.

I have to say, in the way our society is currently set up, this is quite the tricky situation to deal with. It’s true without someone coherent in medial terminology supervising each and every check up, there is no way to ensure that things are being done in a proper way or that the truth is being told. The best measures to prevent Medicare fraud would essentially be company check ins along with surprise surveillance at appointments to ensure everything that they say is being done is being done.

If and when a physician is found “distorting” the situation, both he and the company should be held accountable. This will help ensure stricter guidelines are in place when hiring, which is step one in assuring quality assistance and no Medicare fraud.

Don Berwick on Patient Centeredness

As mentioned in the last blog Don Berwick is Obama’s candidate for the new head for CMS. Berwick’s views on patient centeredness are ones he claims will completely re-orchestrate the medical field. He essentially feels that what is analyzed and what is done to a patient should be ultimately the patient’s decision. If a patient decides that they want an MRI, they should be allowed to get one, even if the doctor doesn’t see it as necessary.

“I contemplate in this a mature dialogue, in which an informed professional engages in a full conversation about why he or she—the professional—disagrees with a patient’s choice. If, over time, a pattern emerges of scientifically unwise or unsubstantiated choices—like lots and lots of patients’ choosing scientifically needless MRIs —then we should seek to improve our messages, instructions, educational processes, and dialogue to understand and seek to remedy the mismatch.”

These words match my own views almost perfectly. The goals of doctors and medical facilities should primarily be educating patients and leading THEM into making wiser decisions. Leave the choice ultimately to the person in which the act will affect! I personally feel all decisions made in regards as to what a person does with their own body and life should be left up to the person whose body and life it is. Of course, this ideal effects a large range of categories, and medicine is definitely a big one of them.

In general I feel education is the answer, and that no person anywhere should blindly follow what they are told by anyone. This means by our government, by our doctor, by our teachers, or by our parents. Learning is not about memorizing what those before you thought and said, but it about UNDERSTANDS what those before you understood, and then building on it. Just because they say it’s true, doesn’t mean it’s true. Just because she’s says you can trust her, doesn’t mean you CAN trust her.

Obama is planning to appoint Don Berwick, A Harvard Pediatrician, a well recognized expert on Health policy, and the head of Institute Healthcare Improvement, as the new head of CMS. This is a much acclaimed decision considering how well known Berwick is throughout the world for his previous efforts on re-engineering medical practice.

Though many people are really excited about this decision there are still many questions and doubts that have been raised, mostly stemming from Berwick’s role as a pediatrician. Can a Pediatrician work well in the field of Medicare and Medicaid? Could he be as motivated to help in this area as he was to help in INI considering the difference of those being worked with and worked for?

I personally feel people are putting too much emphasis on his past. Though it is 100% true that without motivation, productivity drops immensely, as well as the quality of the work, I feel that a person that is interested in “fixing the system” and has the intelligence to do so, will be able to work in any of these fields. The question as to whether or not he will have a “balanced” view is difficult to truly examine as in order to do so we would have to understand what a balanced view is. If you are someone that receives Medicaid or Medicare chances are you are going to view any decision that goes against those fields as negative, biased, or unfair. It is very difficult to remove ones owns biases and see decisions being made from the perspective of the bigger picture. I do feel he has potential to help rework the system in a positive light, but when it all comes down to it, only time will tell.

Technology

We live in times of exponentially improving technology that has already changed our society in ways once barely imaginable. It’s hard to believe that 20 years ago, computers were rarely soon and that 60 years ago television was a new technology.

To take this even further, computers that now exist are millions of times more powerful than their early ancestors of only about 30 years ago. Not only are they becoming more powerful, but their size, portability, and overall usage have improved drastically as well. It is predicted that in 2013 we will have computers that are better processors than the human brain!

In school, information on new sciences becomes outdated in 2 years making half of what is learned during a 4 year program obsolete upon graduation. The new applicable knowledge comes in form of adaptability and the ability to keep up with the ever changing technological world.

It’s hard to even truly believe that the world is changing at these rates and it makes it truly amazing to think as to where we will be in 20 years, in 50 years, in 100 years! To be able to witness these days is truly amazing.

Leaving Your Nursing Home to Get Out On Your Own

There is a fairly new program that has been established by the government that exists solely for two reasons: Helping people in nursing homes live on their own and saving the Government tens of millions in Medicare expenditures. This program is called “Money Follows the Person” and was initially established in 2005. Though the program was established in 2005, most of the 29 states now participating in it didn’t start until the last couple years making this still a pretty new and growing program that plans on having helped moved out 37,000 people by 2013.

Though this goad is admirable though how achievable it is may be up for question. At the end of last year Georgia had only achieve 10% of what it had hoped for and that was the common amount achieved for all the states. The progress may be slow, but the progress made has shown to be quite successful shining a light on the potential of where this could be headed. The slow speed of the process is due to the need to find affordable housing, meeting resistance from nursing homes, and the strict federal guidelines placed for those that can participate in the program.

The healthcare reform has nearly doubled the programs budget and extended the goal to 2016 from 2013 in order to make it more achievable. It has also reduced the eligibility rules greatly helping in finding potential applicants. Even still, the program has a long way to go before it reaches anywhere close to its original expectations.
Even though it is off to a slow start, it is still off to “A” start which is definitely a step in the right direction. There are far too many people in nursing homes that really do not need to be there and this program can help to assure that everybody gets to be where they would like to be and can be and always you can count on SilverCensus to update you every step of the way.
